Pharmacist Referrals

We collaborate with pharmacists in community and hospital settings in the care of complex or challenging patients. Patients likely to benefit from a referral have multiple drug therapies, sub-optimal treatment responses, adherence issues, questions and concerns, or advanced age. We accept referrals for patients with Canadian addresses, including residents in other provinces, regardless of their eligibility for PharmaCare-funded service or mobility issues.

Please include the following with your referral:

  • patient name and contact information
  • personal health number (PHN) (if applicable)
  • name of usual physician and contact information
  • reason for referral
  • notes, consultation letters, lab values and other health information (when available)

Our goal is to process requests within two business days of the referral being received by the Pharmacists Clinic.
